All monthly services include a one time $150 enrollment fee.

    • Monthly subscription service

    • Cost dependent on patient age; $100-$175 per month

    • Includes gender affirming care, addiction recovery options

    • Routine primary care and acute care

    • Chronic condition management

    • Yearly lab panel

    • Medication prior authorizations as needed

    • $300 monthly subscription includes 2 adults and 2 children

    • Add additional children for $60 per month (per child)

    • Includes gender affirming care and addiction recovery options as needed

Supplemental Care

Already have a primary care doctor and needing additional care?

Dr. Liz offers supportive care plans for limited services.

    • Available as a $100 per visit service

    • Introductory evaluation

    • Prescriptions in 3-month increments

    • Necessary referrals with supporting letters if needed

    • Medication prior authorizations as needed

    • $100 monthly subscription service

    • Introductory evaluation

    • Medications for opioid and alcohol use disorder as needed

    • Referrals for ancillary services

    • Medication prior authorizations as needed

    • 60 minute session where Dr. Liz assists you with open enrollment setup

    • $200

    • Vasectomy procedures

    • Skin Biopsies

    • Joint injections

    • IUD and Nexplanon placement / removal
